Parathyroid hormone (PTH), which regulates the extracellular calcium concentration,
is measured in serum or plasma for the differential diagnosis of hypercalcemia and
hypocalcemia and for the investigation and management of parathyroid function in disorders
of mineral and bone metabolism and renal failure. An additional function for PTH measurements
has evolved in the last decade. Modifications to PTH assays that resulted in “rapid,”
“quick,” “point-of-care,” or “intraoperative” assays, has allowed the intraoperative
monitoring of PTH levels as a guide in the surgical removal of hyperfunctioning parathyroid
glands. Assays for PTH also have evolved with the changing knowledge of the PTH forms
in the circulation. This article focuses on PTH measurement and the application of
PTH for intraoperative monitoring.
